To receive an Activation Key for your HearID system, please provide the following details.

Your Name:*
Your Email:*
Business/Institution Name:* Ensure your Business Name is entered accurately, as your Activation Key is bound to it and cannot be modified.
System Number:* Your System Number is on the installation CD label. It has the format:
HID 12345.
Measurement Modules:*

MEPA3
DPOAE
TEOAE

Tick all the Measurement Modules that are activated in your HearID system.
